RESUMEN

Clear cell odontogenic carcinoma (CCOC) is a rare malignant odontogenic tumor. It is characterized by showing, on histopathological examination, clusters of vacuolated and clear tumor cells with epithelial differentiation surrounded by fibrocollagenous stroma and fibroblasts. The present study presents a rare clinical case of mandibular CCOC associated with an impacted tooth in a 26-year-old woman surgically treated with mandibulectomy and reconstruction with iliac crest bone graft. The patient has been followed up for 22 months without signs of recurrence. A search for case report/case series was carried out in the PUBMED database, as well as in the references of relevant previously published literature reviews. Ninety-six publications were identified, totaling 136 distinct cases reported. Female sex was the most affected (63.1%) with 63.3% of cases occurring in patients in the fifth, sixth, or seventh decades of life. The mandible was more affected than the maxilla (74.2%). Association of CCOC with impacted teeth was found in 2.4% of cases, thus rendering it a rare occurrence. The present case report corroborates the results of the survey regarding sex and anatomical location of the tumor; however, it contradicts the findings regarding age predilection. The case described is the fourth known occurrence of tooth impaction associated with the tumor and the first in a female. In conclusion, CCOC should be considered, as well as other malignancies, as a possible diagnosis of maxillary or mandibular intraosseous lesions even in unusual circumstances such as in association with impacted teeth and in young patients.

BACKGROUND: Sialolipoma is a rare histological variant of lipoma commonly misdiagnosed and composed of a proliferation of mature adipocytes with secondary entrapment of normal salivary gland tissue. The purpose of the present study is to report the clinicopathologic and immunohistochemical features of 10 new cases of sialolipomas in conjunction with a review of the literature. METHODS: A retrospective descriptive cross-sectional study was performed. A total of 54,190 biopsy records of oral and maxillofacial lesions from four oral and maxillofacial pathology services in Brazil were analysed. All cases of lipomas were reviewed, and clinical, demographic and histopathological data were collected of all cases compatible with sialolipomas. In addition, immunohistochemistry stains (AE1/AE3, CK7, 34ßE12, S-100, HHF35, α-SMA and Ki-67) and a literature review based on a search of three electronic databases (PubMed, Web of Science and Scopus) were performed. RESULTS: Among all lipomas reviewed, there were 10 cases of sialolipomas. The series comprised of 7 females (70.0%) and 3 males (30.0%), with a mean age of 46.1 ± 21.5 years (range: 11-71 years) and a 2.3:1 female-to-male ratio. The lower lip (n = 3, 30.0%) and tongue (n = 2, 20.0%) were the most common locations, presenting clinically as a nodule of slow growth and normal colour. Conservative surgical excision was the treatment in all cases. No recurrence was observed. CONCLUSION: Sialolipomas are a rare histological variant of lipoma, affecting the salivary glands, mainly in the parotid gland and palate of female adults. Pathologists must recognise sialolipomas to avoid misdiagnoses with other lipomatous tumours that can affect salivary glands.

Introduction: Evaluating the quality of life allows a more accurate clinical control as well as the provision of prognostic information for specific groups. This study was designed to evaluate the quality of life of elderly diagnosed with oral squamous cell carcinoma submitted to combined modality therapy. Methods: This is a retrospective and observational study, with cross-sectional quantitative character. A total of 206 records of patients with head and neck cancer treated between April 2013 and October 2014 were analyzed. Eleven patients over six months of treatment completion were included in the study. The questionnaire of quality of life of the University of Washington (UW-QOL) was applied. The data were collected regarding the social-demographic, clinical-pathological and therapeutic profiles, and the non-stimulated salivary flow was measured. Statistical analysis of quantitative data was performed by the Spearman nonlinear correlation, considering a confidence of 95%. Results: Chewing, saliva and speech showed the lowest scores (31.8; 42.3; 60.6, respectively). Statistically significant correlation was found between: shoulder and mood (r=0.787); swallowing and chewing (r=0.761); completion time of radiotherapy and recreation (r=0.659); activity and recreation (r=0.653); pain and swallowing (r=0.626); chewing and speech (r=0.607); age and speech (r=-0.617). Conclusions: Elderly with oral squamous cell carcinoma diagnosis sunmitted to combined modality therapy presented the areas related to chewing, saliva and speech as the most committed ones. Older individuals have greater impairment of speech, as well as those with longer completion of radiotherapy have better results related to the recreation area. (AU)


Introdução: A avaliação da qualidade de vida permite controle clínico confiável e provisão prognóstica para grupos específicos. Este estudo foi desenhado para avaliar a qualidade de vida de idosos com diagnóstico de carcinoma oral de células escamosas submetidos à terapia combinada. Método: Trata-se de estudo observacional e retrospectivo, de caráter quantitativo com corte transversal. Foi analisado um total de 206 prontuários de pacientes com câncer de cabeça e pescoço tratados entre abril de 2013 e outubro de 2014. Onze pacientes com mais de seis meses de conclusão do tratamento foram incluídos no estudo. O questionário de qualidade de vida da Universidade de Washington (UW-QOL) foi aplicado. Foram coletados dados relacionados ao perfil sociodemográfico, clínicopatológico e terapêutico, e foi mensurado fluxo salivar não estimulado. A análise estatística dos dados quantitativos foi realizada por meio da correlação não linear de Spearman, considerando uma confiança de 95%. Resultados: Mastigação, saliva e fala apresentaram os menores escores (31,8; 42,3; 60,6, respectivamente). A correlação estatisticamente significante foi observada entre: ombro e humor (r=0,778); deglutição e mastigação (r=0,761); tempo de conclusão da radioterapia e recreação (r=0,659); atividade e recreação (r=0,653); dor e deglutição (r=0,626); mastigação e fala (r=0,607); idade e fala (r=-0,617). Conclusões: Idosos com carcinoma oral de células escamosas, submetidos à terapia combinada, apresentaram áreas relacionadas à mastigação, à saliva e à fala como as mais acometidas. Indivíduos mais velhos apresentaram maior comprometimento da fala, assim como os que apresentavam maior tempo de conclusão da radioterapia apresentaram melhores resultados relacionados à recreação. A radioterapia é uma modalidade terapêutica eficaz utilizada no tratamento de tumores malignos da cavidade oral. Sua utilização em região de cabeça e pescoço levaao surgimento de efeitos adversos.A mensuração da qualidade de vida permite reunir os problemas mais comuns aospacientes com câncer oral de forma estruturada, classificando sua intensidade, tornando-se possível um controle clínico mais apurado. O presente estudo teve como objetivo avaliar a qualidade de vida de pós-irradiados com diagnóstico de carcinoma espinocelular de cavidade oral. Onze pacientescom mais de seis meses de conclusão do tratamento, atendidos no Hospital Haroldo Juaçaba –Instituto do Câncer do Ceará, foram incluídos no estudo. Foi aplicado o questionário de qualidade de vida da Universidade de Washington, específico para o câncer de cabeça e pescoço. Foram coletados dados relacionados ao perfil sociodemográfico, clínico-patológico e terapêutico. Realizou-se a mensuração do fluxo salivar não estimulado através do método de spitting. Dados categóricos foram expostos em forma de frequência absoluta e percentual e os dados quantitativos em forma de média ± desvio-padrão, seguido de sua mínima e máxima. Radiotherapy is an effective therapeutic modality employed in the management of malignant tumors in the oral cavity. Its use in head and neck results in adverse effects.The measurement of quality of life allows to gather the most common problems in oral cancer patients in a structured way, ranking its intensity and enabling a more accurate clinical control.
